Applying For A United States Passport Name Change

If you have gotten married or divorced recently, you will need to change your last name on several personal documents. Of these documents, one of them is likely to be your passport. Submitting a Passport Name Change can be fairly simple to do although it can take some time to complete the full process.

The first document you will need to have your name changed on will be your Social Security card. Until you have a Social Security card with the correct name, you cannot change your passport. Contact your local branch of the Social Security Administration or visit them on the Internet to obtain additional information on how to change your Social Security card.

Most people need to use form DS-82 to submit a passport name change. This form is usually available at your local post office. If they do not have it or you want to print it from the convenience of your own home, the Bureau of Consular Affairs has it on their website so that you can print it out. This form is the correct one if it has been longer than a year since receiving your last passport.

Along with this form, you are required to also submit legal proof of your name change. For example, if you were recently wed, you will include a copy of your marriage certificate as proof. You also must have been a minimum of 16 when you received it and the passport must not be any older than 15 years.

Once you have filled out the form, before you can send your information in, you will need to have passport photos taken. You can do that one of your neighborhood drug stores. If you check with them they typically offer passport photos for a fee. You can also take your own passport photo but it must measure 2 inches x 2 inches. Any other size will be rejected.

Now it is time to mail your application to the address on the document. You need to place all of your documentation along with the application and fee into an envelope that is padded. You should consider using a method of mailing that can be traced so that you can be sure it was received. The fee you will pay to change your name will be the same fee as the renewal of an adult passport.

If you will need your passport quickly, there is an additional fee of $60 to have it expedited. For the fastest possible delivery, it is a good idea to mark the outside of the padded envelope with the word "EXPEDITED." This helps to ensure that those who handle it will treat it as a rushed package. Alternatively, if you need it in less than 2 weeks you can also make an appointment with a Regional Passport Agency.
