It is always a smart thing to have an emergency number list for yourself as well as for babysitters. Laminate, frame or just post it up somewhere in your home that is easy to get to in the event of an emergency. Here are some things you should have on your sheet but feel free to personalize your sheet with information that is specific to your families needs.
* Where You can be reached i.e. the location...movie theater, restaurant, etc
* Address and Phone Number for Location
* The time you expect to be returning home
* Who to call if there is an emergency and you are unable to be reached- Name, phone number and relationship with your child.
In case of an emergency the sitter should know the following information:
* Your Home Address
* Name of Subdivision (if applicable):
* The Closest Intersection or Main Street in case Emergency Personnel ask
* Name of Closest Neighbor that can be contacted for help
* Child's Doctors Name, Phone, Office Location, Directions to Doctors Office
* Directions to the Nearest Hospital
* The Emergency Room Number to the Nearest Hospital
* Provider of the Child's Insurance, Group #, Policy ID#:
Always have the following emergency phone numbers posted
* 911 or the local emergency number
* Poison Control: 1-800-222-1222
* Hospital Name and Phone
Information about your kids
* First, Middle, Last Name
* Date of Birth
* Weight (as of the date you will be having the sitter)
* Height
* Medical Conditions (if any)
* Allergies (if any)
Other Information about the kids routines
* What to feed the child and when
* What the child can not eat
* What the child can/cannot watch on Television
* Things your child likes to do/play
* Things not allowed to do/play
* Bedtime Routine
* Bedtime
All of these things will be helpful in the stressful event of an emergency and to help ease your mind that your child are in safe and informed hands.
